This is beyond the scope of MRIcroGL. It is an orthographic renderer. For these sorts of situations, you would likely want to export the MRI brain surface and lesion drawing to a mesh format using a mesh format https://rordenlab.github.io/nii2meshWeb/ I would then consider photogrammetry to create a realistic image of the individuals head (Apple products have lidar enhanced photography and the App Store has terrific tools). You can then align these two modalities.
