dcm2image: support substitution of image parameters into output file names

Detailed description

---- Reported by torsten@synapse.sri.com 2008-06-09 08:32:17 ----

The tool should allow substitution of imaging parameters into the output file
names to resolve situations where scanners may have written images in a
different order in one session than another.

At the very least, we should be able to put TR/TE into the file names. Ideally,
we will also be able to identify protocol names etc., and it would be very
desirable to distinguish magnitude from complex/real and complex/imaginary
images.

Finally, maybe we can detect the phase shift sometimes executed by the scanner
in the i/r images from the DICOM tags and encode it in the output file name?

Partly done: dcm2image can now encode Tr and TE as well as Ge's tags for
r/i/m/p image type. Still no clue regarding phase flips.
